I keep going back to a handful of good games, lately its been the excellent Civ IV (Beyond the Sword) specifically the Fall Further Mod.  It's seeped in backstory and had its genesis as a Dungeons and Dragons campaign world.  The team has put a lot of work and love into it, and it shows.

You can find a Fall From Heaven Wiki here and the Civfanatics Forum section here.

Where most of the time I'd have to shell out $$ for an 'expansion' or a 'new' game using the same engine, this is a fan mod, so its free.

Now its also made by committee, and has to do some shoehorning to work in Civ IV's engine, so I won't say its to everyone's taste, or flawless.  You'd have to enjoy the underlying Civ IV gameplay to get anything out of this, and have a taste for dark fantasy.

So far I've played:

The Bannor - they've -literally- been to hell and back, and its left its mark on their psyche.  They're a good choice for noble crusading paladins or foaming at the mouth religious fanatics.  Shatner's Unwashed Crusade AAR is a great, great read.
